Memorial Donation Form

To make a memorial donation, print and complete the form above and mail or bring it to the library with the donation amount.  A letter identifying the gift and donor will be sent to the designated family member(s), and a page describing the donation will be created for our Memorial Book.  The average price of a hardback book is $18.  You can request a title or bring a book you have purchased.  Books are subject to HISD collection development policy.  Picture books must be in hardback format.
